From Mandrin

• Head east on Orange Picker Rd toward Corner Oaks Dr and pass by Wells Fargo Bank (on the right in 0.5 mi)
• Turn left onto FL-13 N/San Jose Blvd
• Pass by Talbots (on the right in 2.9 mi)
• Turn right onto Crown Point Rd
• Turn left onto Old St Augustine Rd
• Altitude Health will be on the right

From Orange Park

• Follow Kingsley Ave to Hwy 17 N/Park Ave
• Head south on Plainfield Ave toward Kingsley Ave for 102 ft.
• Turn left at the 1st cross street onto Kingsley Ave for 0.3 miles
• Continue on Hwy 17 N/Park Ave. Take I-295 S to FL-13 N/San Jose Blvd in Jacksonville.
  • Take exit 5 from I-295 S for 6.4 miles
